Rising Health is a functional and integrative medicine clinic providing care in Holladay, Utah and the Salt Lake valley. These Terms and Conditions govern your use of risinghealthutah.com, our website chat and any services offered through them (together, the “Services”), including any text messages you may receive from us.

By accessing or using our Services, you agree to be bound by these Terms.

We may change these Terms at any time and will post the updated Terms on this website. Your continued use of the website means you accept the Terms in effect at the time. We recommend checking this page from time to time.

Use of the website

Business identity

Rising Health Specialty Clinic (Rising Specialty Medical Center, LLC) sends text messages to keep you informed about consultation follow-ups, appointment reminders and confirmations, care updates, clinic news, events and offers, and other communications related to our services. Program name: Rising Health Specialty Clinic Text Messages.

How you opt in

You opt in through our website chat, which has two separate checkboxes: one for care and appointment messages, and one for marketing messages. Neither is checked for you, and you can use the chat without checking either.

Message frequency

As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive recurring messages, with frequency varying based on your engagement status, appointments and preferences. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.

Opt-out instructions

You can cancel the SMS service at any time. Just text “STOP” to +1 801-419-0705. After you send the SMS message “STOP” to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.

Health information by text

Appointments and services

Consultations and scheduling

Free phone consultations are scheduled with a patient advocate. We work to keep scheduled appointment times, but can't guarantee availability within a specific window.

Clinical services

Clinical care is provided after an evaluation and is governed by our separate patient intake forms, consent forms and financial policies, which you'll receive before treatment.

Intellectual property

All content on our Services, including logos, images and text, belongs to us or our licensors and is protected by intellectual property laws. You may not reproduce or distribute it without our written permission.

Reviews and feedback

If you send us feedback or content, you grant us a non-exclusive, royalty-free license to use, display and reproduce it. We will never publish anything that identifies you as a patient without your permission.

Disclaimer of warranties

Our website and chat are provided “as is” and “as available.” We make no warranties, express or implied, about their operation or availability.

Limitation of liability

To the fullest extent permitted by law, we will not be liable for any damages resulting from your use of, or inability to use, the website or chat.
